Mr. Nathanial H. Duck, a representative of Dstar Productions and a renowned physical education trainer and nutritionist, would like to make a video of our Preschool children performing and experiencing a fun, physical and educational training session. Dstar Productions will be filming this exciting fitness video on Thursday, September 26, 2013, in the FKO Preschool Room.
Please review the information below and sign the Permission Slip: By signing this permission slip, I am giving my permission to Dstar Production to utilize photos and videos of myself and/or my child(ren).
All media inquiries, print material, photos or videos, are under the exclusive copyright of Dstar Productions, CEO Nathanial H. Duck. I understand that I (the parent/guardian) will not receive compensation of any kind (financial or otherwise) for the use of any video and/or audio portion of mine and/or my child’s individual or group training session that may be used by Nathanial H.
